If CMI Tools is loaded but you don't see the toolbar, right-click your mouse over some blank toolbar space as shown here:
 

Browse to the CMI Tools toolbar and click on it to make visible.

When you switch workspaces, the CMI Tools toolbar may disappear.  To save your toolbar settings when you switch workspaces, type wssettings at the command line, and toggle on "Automatically save workspace changes".

To add the CMI Tools pull-down menu, open the CUI by typing cui at the command line:

Choose the workspace to which you would like to add the CMI Tools menu.  In this case, the Land Desktop workspace is chosen.  Click on the Customize Workspace button at the upper right side:
 

Scroll down to the CMITOOLS partial cui file on the left.  Expand CMITOOLS to view the CMI Tools menu, and toggle it on.  Click on the Done button at the upper right, and click OK.  You can do these for each of the workspaces that you use.
